Position: Case Manager

Key Responsibilities:

As a Case Manager, you will:

Case Management & Care Coordination

Conduct comprehensive assessments to understand residents' physical, emotional, and social needs.

Develop and implement Individualized Care Plans (ICP) to provide tailored support.

Coordinate with medical professionals, therapists, social workers, and external agencies for holistic care.

Monitor, review, and adjust care plans to ensure effectiveness.

Resident Welfare & Advocacy

Act as an advocate for residents, ensuring their rights and well-being are prioritized.

Provide counseling and emotional support to residents facing challenges.

Facilitate communication between residents, families, and staff to enhance care delivery.

Program & Service Management

Organize, manage, and evaluate programs and activities to enhance residents' engagement and well-being.

Work collaboratively with community partners to provide additional support and resources.

Participate in training, supervision, and team meetings to ensure best practices in case management.

Emergency Response & Crisis Intervention

Assist in handling crisis situations such as medical emergencies and behavioral incidents.

Implement and support emergency contingency plans to safeguard residents.

Documentation & Compliance

Maintain accurate case records, care plans, and reports.

Ensure compliance with organizational policies and regulatory requirements.
