Responsibilities- You will coordinate care and case management for patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans while ensuring seamless care coordination and continuity.
- You will collaborate with multidisciplinary teams, including physicians and allied health professionals, to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources.
- You will support population health research through data collection, collect treatment data and facilitate stakeholder meetings, and with various stakeholders.
- You will organise population health initiatives for the community and ensure all programmes comply with relevant regulations and standards, ensuring all works meet regulatory compliance.
- Diploma with 8 years' relevant experience or degree in a healthcare-related field
- Experience in care coordination, case management, or a similar role
- Knowledge of community resources and services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
