You will support clinics under the Primary Care Network (PCN) and collaborate with various stakeholders to ensure timely delivery of care coordination efforts. You will manage care delivery challenges alongside the PCN team, participate in Quality Improvement and Primary Care Partnership initiatives, and handle PCN programme reporting and claims administration. Additionally, you will provide support for any other projects or tasks as assigned.
**Requirements**:
- Degree with at least 2 years’ experience
- Excellent written and verbal communication skills
- Ability to work independently and collaboratively with multi-disciplinary teams at all levels
- Meticulous and self-motivated team player who is able to multi-task under tight timelines
- Proficient in data analysis, budgeting, and secretarial duties
