Senior / Care Coordinator, Population Health & Integrated Care

Overview
You will manage patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans whilst ensuring seamless care coordination and continuity. You will collaborate with multidisciplinary teams including physicians and allied health professionals to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources. You will also collect treatment data and facilitate meetings with various stakeholders. Additionally, you will organise population health initiatives for the community and ensure all programmes comply with relevant regulations and standards.
Responsibilities
Manage patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans to ensure seamless care coordination and continuity.
Collaborate with multidisciplinary teams including physicians and allied health professionals to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources.
Collect treatment data and facilitate meetings with various stakeholders.
Organise population health initiatives for the community and ensure all programmes comply with relevant regulations and standards.
Qualifications
Diploma with 8 years’ experience or degree in a healthcare-related field
Experience in care coordination, case management, or a similar role
Knowledge of community resources and services
Excellent communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
