Our organization seeks a dedicated Care Coordinator to facilitate seamless patient care and foster collaborative relationships among multidisciplinary teams.
Key Responsibilities:- Manage complex patient cases by maintaining comprehensive records of demographics, medical histories, and treatment plans.
- Collaborate with physicians, allied health professionals, and other stakeholders to coordinate care plans, manage referrals, and ensure timely access to community resources.
- Collect and analyze treatment data to inform decision-making and facilitate stakeholder engagement.
- Develop and implement population health initiatives that align with regulatory standards and organizational objectives.
To excel in this role, you should possess excellent communication and interpersonal skills, as well as experience in care coordination, case management, or a similar field.
