Service Coordination and Advice

**Overall Job Responsibility**:
- Enhance clients’ service journey and ensure that persons with disabilities (PwDs) with complex needs can receive dedicated help from the Service Coordination and Advice (SCA) Division through the following:
- Provide person-centred support through service coordination and advice to persons with disabilities (PwDs) and their caregivers/ families
- Support continual service review and enhancement
- Foster stakeholder relationships
- Support the Division executing any other strategies related to operations and service delivery in support of SG Enable’s objectives
**Duties and Responsibilities**:
**Provide person-centred support through service coordination and advice to persons with disabilities (PwDs) and their caregivers/ families**
- Assess and establish needs of PwDs and their caregivers/families, provide information and advice on appropriate services, programmes or schemes that may meet their needs
- Process referrals to government-funded services (e.g. MSF funded programmes)
- Conduct assessment and right siting of PwDs to suitable government funded services or programmes
- Provide case coordination support for vulnerable PwDs/families, linking up and coordinating care support with other agencies as needed
**Support continual service review and enhancement**
- Conduct regular reviews and enhancement to work processes and practices
- Engage relevant stakeholders (such as govt agencies, social service agencies and other public institutions) to define, prototype and test the new workflows and SOPs
**Foster stakeholder relationships**
- Establish and maintain strong partnerships with stakeholders
- Manage processes of information flow across stakeholders and platforms to ensure that information and advice shared with PwDs/caregivers is accurate and up-to-date
- Coordinate cases with the stakeholders effectively as required, ensure smooth flow of information
- Organise and facilitate discussion/coordination sessions
- Surface feedback on policy gaps to relevant agencies as required
**Qualifications, Knowledge and Experience**:
- Bachelor’s degree in any discipline
- Minimal 1 year work experience in social service sector is preferred, particularly in child / adult disability services. Fresh graduates with the necessary skills and traits are also welcome to apply
- Keen interest in social services sector. Understand / Willing to learn about the different types of disabilities, their implications on functions across life stages, and the typical experiences faced in navigating the disability landscape
- Good understanding of person and family-centred approaches
- Good active listening skills and empathy
- Strong communication skills and social etiquette
- Keen eye for detail
- Proficient in MS Office and preferably CRM systems
