**Overall Job Responsibility**:

- Enhance clients’ service journey and ensure that persons with disabilities (PwDs) with complex needs can receive dedicated help from the Service Coordination and Advice (SCA) division through the following:

- Provide person-centred support through service coordination and advice to persons with disabilities (PwDs) and their caregivers/ families.
- Co-lead on continual service review process and enhancement.
- Build professional partnerships and collaborations
- Support the Division executing any other strategies related to operations and service delivery in support of SG Enable’s objectives

**Duties and Responsibilities**:

- Conduct needs and intake assessment of PWDs and their families/ caregivers, identify appropriate support and develop care plan to address the identified needs and concerns
- Provide case coordination by working collaboratively with other professionals to mobilize community resources to support and address the identified needs of PWDs and their families/caregivers
- Attend to the enquiries and provide relevant information and referral services, appropriate schemes that will address the needs of PWDs and their families/ caregivers
- Assist in processing and facilitating referrals to the government-funded adult disability services
- Review and conduct closure of cases upon achieving sustainable service outcomes for clients and the families
- Maintain proper and up to date case notes and documentation
- Conduct case review sessions with staff as part of good case management practice and development for the team
- Review, support and implement streamlining of the operational guidelines, data management process and any other business processes as required with the view to continuously improve work practices
- Assist with monthly data tracking and reporting to the management level
- Develop plans to build and sustain stakeholder engagement and relationships
- Advocate for needs of PwDs and their caregivers
- Organise and facilitate discussion/coordination sessions
- Provide guidance to staff by ensuring that the team maintains all relevant records/documentation/case notes in accordance to the SOPs and guidelines

**Qualifications, Knowledge and Experience**:

- Bachelor’s degree in any discipline. Relevant qualifications in social work, allied health profession (such as Occupational Therapy), psychology will be an advantage
- Minimum 5 years of work experience with 1 year of supervisory experience in social service sector is preferred, particularly in child / adult disability services
- Keen interest in both direct and indirect practice in social services sector. Strong interest and passion to serve people with multiple disabilities and understand different types of disabilities and the implications on functions across life stages, and the typical experiences faced in navigating the disability landscape
- Excellent organisation skills and time management skills
- Active listening skills and empathy
- Effective communication skills and social etiquette
- Keen eye for detail
- Proficient in MS Office and preferably Microsoft CRM
