Executive / Senior Executive, Advance Care Planning

The Executive / Senior Executive will be responsible in assisting the Advance Care Planning (ACP) clinical and operations leads in the daily running and maintenance of the ACP programmes in the institution, and ensure continuous improvement to work processes.
**MAJOR DUTIES AND RESPONSIBILITIES**
**(A) SPECIFIC**
1. Coordinate and support ACP awareness talks, roadshows and ACP trainings in the institution.
2. Monitor key performance indicators and ensure optimum utilization of resources.
3. Work closely with ACP Facilitators on issues relating to ACP workflows and services.
4. Work with partners in the intermediate and long term care sectors as well as other service providers.
5. Work with ACP National Office on the quarterly budget, KPI reports and audits.
6. Plan and execute events and projects for continued outreach efforts.
7. Participate in ACP-related research in the institution.
8. Provide secretariat support to the ACP Committee in the institution.
9. Supervise and work with the ACP Administrative Assistant (AA) on:
a. Preparation and maintenance of ACP documents in the institution, and ensure information and documentation are updated for facilitators in the inpatient and outpatient settings
c. Maintenance and updates of the ACP patients’ database
d. Maintenance and up-to-date database of trained ACP facilitators and ACP trainers
e. Coordination with patients for ACP facilitation within the institution
**(B) GENERAL**
1. Align the department's goals with the business goals of the hospital.
2. Plan and execute assigned projects
3. Work in collaboration with key stakeholders in the evaluation and quality improvement processes on the ACP programme and projects.
**JOB REQUIREMENTS**
EDUCATION, TRAINING AND EXPERIENCE
- Basic Degree
- Minimum of 2 years in healthcare or social service related areas preferred
- Possess positive attitude, initiative and keenness to learn.
- Possess strong analytical, good interpersonal and communication skills
- No experience in ACP required as training will be provided
- Knowledge of computer software, MS Office
- Knowledge of statistics
**ABNORMAL PHYSICAL WORKING CONDITION**:
- Staff may be required to travel to patients’ homes, nursing homes, hospices, other step-down care units or other locations as needed to conduct ACP discussions or advocacy activities
- Staff may be exposed to patients and visitors of various psychological and emotional states and who possess varying personality traits.
