**COMMUNITY CARE EXECUTIVE PROGRAMME (CCEP)**

Our ComCare Executive Programme (CCEP) offers a three-year structured development pathway specially designed to nurture you to be the future generation Community Care leader.

You will join as a ComCare Executive Associate and undergo an accelerated learning journey and experience first-hand exposure to the eldercare landscape. You will learn about St Luke’s ElderCare’s delivery of our full range of integrated services and programmes for our elders, which include maintenance day care, day rehabilitation programmes, dementia care, nursing care, respite care, and wellness programmes. These services are designed to benefit elders who have suffered physical disabilities due to stroke, falls or accidents, as well as befriend and empower the frail and lonely who need care and support. You will also be offered opportunities to fast-track your career and broaden your perspective, grow expertise, and to prepare yourself to assume significant responsibilities in the sector.

Programme Description:

**Responsibilities**
- Job rotations in Day Care and Residential Care settings
- Provide quality service to eldercare clients
- Delivery of basic nursing tasks and maintenance therapy
- Support daily operations smoothly and perform incident management protocols
- Selection of career track in either clinical or non-clinical track from second year onwards based on domains of Care Management/Programme Management/Operations Management/Partnerships and Community Relations:
Care Management
- Conduct health assessments
- Develop individualised care plan and carry out the care plan
- Conduct activities suitable for seniors
- Ensure compliance of medication administration

Programme Management
- Conduct programme assessments to tailor active ageing plan for each elder
- Collaborate with other community partners to develop programme and services
- Review programmes for effectiveness
- Promote programmes and services

Operations Management
- Maintain upkeep of centre facilities
- Ensure the safety and security of the centre
- Develop schedule for centre operations and programmes, transport operations and manpower planning
- Coordinate for client referrals and admissions
- Generate and review operational reports

Partnerships and Community Relations
- Support the needs and well-being of the community through home visits, care planning, service referrals and activity recommendations
- Establish partnerships to grow programmes and services
- Manage partners and volunteer groups and their activities and programmes

**Requirements**:

- Good diploma in nursing, community care, social services, health promotion, business, management, arts & social science, communications, logistics, and other equivalent disciplines
- Passion for eldercare/healthcare
- Deep sense of ethics
- Strong ability to work and learn independently
- Strong innovation drive
- Excellent Co-Curricular Activities track record
- Volunteer experiences will be an advantage
