- OtherAdvance Care Planning (ACP) enables clients and patients to discuss their care preferences with loved ones and care providers ahead of a medical crisis.

The Advance Care Planning (ACP) Assistant Manager/Senior Executive will be a part of the ACP National Office at AIC, supporting and driving the implementation of a national ACP programme in Singapore.
- Drive the engagement and recruitment of new partners in the health and community care sector to increase touchpoints for Advance Care Planning.
- Undertake role of Programme Manager (which includes fund administration) for assigned stakeholders, working collaboratively with them to achieve desired programme outcomes
- Provide secretariat support for ACP related workgroups and committees
- Support IT administration and projects
- Support capability development and training of external providers
- Co-develop and implement public education and outreach initiatives to raise ACP awareness

**Job Requirements**:

- Degree in any field
- Prior experience in acute care or community settings and/or a project management role would be advantageous.
- Possesses strong interpersonal and communication skills (written and spoken), knowledge in data analytics and is meticulous
