**Overview**

This programme coordinator will be responsible for the coordination of program requirement with the identified suitable patients and consultants. He/she will be required to manage equipment loan or issued to the patients for program's monitoring.

**Responsibilities**:
You will be responsible for the following:
**Primary Duties and Responsibilities**
- **Operations & Case Management**
- Discuss with Neurologist Stroke Registrar/ Consultant to identify suitable patients, their target BP range and the anti-hypertensive medication use.
- Introduce the purpose of service to patient and obtain signed consent through the VSM Programme Participation Form.
- Obtain and scan acknowledgement loan chit for devices.
- Assist the patient in downloading the mobile VSM apps and guide patient with the use of VSM dashboard (i.e. connecting to Bluetooth; inserting BP readings; and troubleshooting) before patient discharged.
- Perform basic troubleshooting with patients, and refer to vendor if necessary.
- Document service details including target BP, duration of service, discharged anti-hypertensive agents, name of referring doctor-in charge for escalation purpose and consent.
- Ensure referral to Carehub team for subsequent monitoring in place.
- Conduct patient and family education on the use of BP meter and proper technique of BP measurement.
- Collect and submit data for the purpose of evaluating service outcome.
- Act as resource person for nurses and doctors within the unit and institution for VSM care related issues.
- Collect and safe keep devices when return.
