ABOUT THE ORGANISATION:
The Health Promotion Board’s (HPB) vision is to build a nation of healthy people. We want to empower residents in Singapore to attain optimal health, increase their quality and years of healthy life, and prevent illnesses, disability and premature death. To achieve this, HPB drives national health promotion and disease prevention programmes, and creates a supportive environment in Singapore where healthy lifestyle options are available and accessible to residents every day.
**ABOUT THE DIVISION**:
To support the Board’s goals, the Regional Health Systems and Community Outreach Division collaborates closely with the community and healthcare partners to co-create regional and community-owned solutions to ensure good coverage and sustainable delivery of priority health promotion programmes in the community setting.
Key programme areas include active ageing, physical activity, nutrition, substance abuse and mental health. The initiatives cover the general population segments from children to seniors, as well as targeted outreach initiatives to ethnic minority groups and disadvantaged groups (such as those from lower socioeconomic status).
This person will look into the collaborations with the whole of government, healthcare partners in identifying the gaps in services and work processes and standards, and help build providers’ capabilities and capacity in the area of preventive health with the support from HPB. He/she will also build partnerships and link up partners into healthy ecosystems so that partners are able to “plug and play” the preventive health bundle programmes in their communities
SCOPE OF WORK WILL INCLUDE BUT NOT LIMITED TO:
Provide guidance to healthcare cluster on-ground teams to align delivery of programmes and services with national standards in support of Healthier SG:
- Provide leadership for the (Regional Health Systems) RHS liaison team
- Lead for strategising, improving Population Health efforts within 3 RHS clusters in Healthier SG
- Design and co-lead studies with healthcare clusters to understand customer needs and key drivers and motivators of target segments, to improve preventive health within healthcare clusters
- Identify how technology can support population health effort across settings
- Use IT solutions to analyse data and recommend actionable insights to guide strategic and operational decisions in delivering population health efforts
- Advise organisational teams to design and work on customer-centric programmes within each RHS Cluster
- Develop processes to capture, track and report on effectiveness
- Provides trend analysis for improvement of RHS specific population health efforts to internal and external partners especially the RHS Office(s) and MOH Office for Healthcare Transformation (MOHT)
- Prepare regular updates to management on performance of key population health metrics
- Accountable for budget management and risk management
REQUIREMENTS AND QUALIFICATIONS
- Previous working experience as an Outreach and Partnerships Manager for at least 6 years;
- A very good understanding of partnership development and relationship management. Ability to network easily and build strong collaborations with stakeholders;
- Excellent communication and interpersonal skills and versatility in dealing with different types of partnerships;
- Strong organisational and time management skills; good multitasking skills but with attention to details;
- Ability to work independently, and with teams;
- Passion to serve, particularly in the community setting
