As a member of NHG’s Group Integrated Care (GIC), the Clinical Programme Manager will play an integral role in clinical integration, which entails strategic planning, project planning, development and implementation involving NHG’s Integrated Care Organisations (ICOs) & primary care, so as to achieve population health goals.
- In facilitating NHG’s goal of adding years of healthy life for its patients & population health goals, he / she is expected to oversee clinical operations, administrative & financial functions of integrated care initiatives for the NHG Diabetes and Metabolic Steering Committee (DMSC) and its workgroups, along with responsibilities in facilitating agreement on programme objectives, deliverables and monitoring performance against targets, key performance indicators and international benchmarks. Hence he/ she is expected to be a good team player, working closely with various institution / ICO’s clinicians and administrators, as well as liaising and collaborating with stakeholders such as the Ministry of Health (and its agencies), government agencies and other partner care providers in the community.**Major Duties & Responsibilities**
- Plan, implement and lead/ manage cluster projects (e.g., DEFINITE Care, NHG Integrated Diabetes Care Programme, etc.) under the purview of NHG Diabetes and Metabolic Steering Committee (DMSC), working closely with a multi-disciplinary care team and various stakeholders to ensure project milestones, KPIs, claims & reporting, and budget utilisation are on track.
- Work collaboratively across team-GIC in facilitating the anchoring of integrative care models in appropriate setting across various clinical horizontals impacting significantly on NHG’s population, starting with the DMSC for the diabetes horizontal.
- Serve as a resource person and maintain positive relationships with stakeholders; plan and prepare annual work-plans, budget requirements and seek approval from relevant authorities.
- Prepare periodic progress reports and fund utilisation to DMSC, workgroups and Funders.
- Plan/ lead and resource annual community education and outreach initiatives
- Oversee and provide secretariat support to project teams. This includes but not limited to meeting coordination and working with Project Lead(s) to curate meeting agenda, draft minutes of meeting, follow up on matters arising, and project/programme planning.
- Undertake any other tasks and duties as assigned by the Supervisor.

**Job Requirements**
Education/Experience
- Minimum degree in healthcare or relevant disciplines.
- Preferably with at least 5 years of relevant working experience in healthcare with at least 3 years of experience in clinical services development / integration.
- Preferably possess strong administrative, budget management, event/ project coordination and management skills. Having Project Management Professional (PMP) certification will be an advantage.
- Preferably possess working knowledge of healthcare utilisation principles.
