Position: Executive Asst (Connected Care Specialist)

Department: Hopes Programme

Job Description:

To be part of a dynamic digital mental health service piloting a novel model of care in psychiatry.

Job Responsibilities:

  • Review eligibility of persons referred into the service.
  • Onboarding of eligible persons into the digital platform.

  • Monitor the health status of persons in the service remotely.

  • Engagement with persons enrolled in the service
  • Follow up with patients and escalate to the assigned healthcare worker if needed, as per stipulated care protocol.
  • Ensure critical information is communicated within the team.
  • Administrative support.

Requirements:

Competency

  1. Prior experience in mental healthcare and/or digital technology will be an advantage.
  2. A passion for mental health

Educational and Professional Requirements

  1. Diploma in any discipline.
  2. Proficient in Microsoft Office applications
