Job Description & Requirements

The Community Outreach Team (CREST) serves as a mental health first-stop touchpoint. CREST promotes early identification of at-risk individuals and provides psychosocial intervention to support those with mental health needs and their caregivers in the community.

Responsbilities

· Promote early recognition of signs and symptoms of mental health conditions and/or dementia.

· Improve mental health conditions and/or dementia literacy by organising targeted outreach events and activities to provide mental health and/or dementia information and education to residents and caregivers in the region.

· Provide psychosocial intervention such as techniques to reduce the risk of escalation of mental health needs.

· Provide collaborative and coordinated care for clients and caregivers through a case management approach.

· Work in partnership with health and community care partners and key stakeholders to co-manage and support persons with mental health conditions and dementia in the community.

Programmes

· Work closely with key stakeholders to plan, organise and implement programmes and activities for residents in the West (Jurong West, Pioneer, Nanyang, Ayer-Rajah & West Coast) and Central (Potong Pasir & Braddell Heights) regions.

· Churn out monthly and quarterly reports

· Craft proposal and apply for relevant funding from external source

· Take in enquiries from the public on participation in current programmes

· Establish and support the run of caregiver support groups

Ancillary Duties

· Support the operations of Viriya Mental Wellness Hub located at Kent Road

· Viriya Children & Medical Programmes: processing the approved applications into excel database, collating bimonthly report to finance department for disbursement to clients

· Planning and executing events within the organization

· Work with stakeholders and caseworks for ad-hoc external programs
