Join Our Team as a Social Work Assistant

Principal Responsibilities

  • Conduct phone screenings to collect biodata and gather social information for medical social workers. Contact patients for appointment reminders and rescheduling.
  • Accompany medical social workers on field visits or conduct them independently.
  • Timely completion of case recordings.
  • Liaise with external agencies .
  • Transcribe applications onto online referral systems.
  • Monitor and follow up on referral statuses.
  • Handle external correspondences.
  • Attend to Medical Social Services enquiry emails.
  • Maintain work statistics and perform clerical duties.

Preferred Qualifications

  • Minimum 1 year of relevant experience in social services or healthcare is preferred
  • GCE 'A' or 'O' Level with passes in English and Mathematics
  • Certificate/Diploma in community and social services or equivalent preferred
