**Principal Responsibilities and Duties**:

- Manage the operation of TOUCH Home Care and oversee all services delivered. These services include meals on wheels, transport and medical escort, and home personal care.
- Monitor client load and programme capacity, staff-client ratios and ensure client muster is as close to optimal levels for quality care and programme efficiency as possible.
- Ensure there is proper case management of all clients, with a holistic and multi-disciplinary approach of care.
- Ensure that all facilities within the centre is properly maintained and safety standards & procedures are in place.
- Provide leadership to the team, facilitate a culture that align to Organization’s core values, and ensure all staff adhere to the Organization policies and procedures in their daily works.
- Work closely with internal specialist (such as the head of nursing, head of therapist, head of transport and head of home personal care) to ensure that staff have the right competencies to deliver services and that clients’ needs are effectively met.
- Keep abreast of the developments of external landscape and provide strategic recommendations to ensure the services are relevant and effective.
- Engage and develop relationship with internal and external stakeholders to mobilize and garner resources within the assigned precinct to build community wellness and provide support for the frail elderly.
- Actively participate in Organizational Development projects.
