To effectively manage cases receiving foster care services and to support programmes and events of the Fostering Department.
JOB RESPONSIBILITIES
- Manage cases receiving foster care services and support programmes or events of Fostering Department and the agency.
- Conduct in-depth assessment and prepare Foster Parent Assessment Report (FAR), and present findings at the CIC Approving Panel and follow up on decisions and recommendations.
- Conduct regular CANs assessments to assess how the foster child is doing as well as the foster parents capability in meeting the needs of the child.
- Conduct necessary statutory visits to ensure that the placement is able to safeguard and promote safety, stability and wellbeing.
- Organize and participate in inter-agency efforts to support the welfare and development of the foster child or CYP.
- Provide a 24-hour hotline for foster parents or kinship care per duty schedule and follow up on outcomes.
- Support foster placement to prevent breakdowns in existing care arrangements for the foster child.
- Participate actively in case management.
- Identify at-risk situations and develop strategies and intervention plan to address child safety issues and wellbeing.
- Monitor and evaluate the effectiveness of case management and other programme and activities extended to foster or kinship care families.
- Submit accurate operation reports, case reviews, discussions to Boys' Town and other governing bodies.
Job Requirements
- Bachelor of Social Work or Graduate Diploma in Social Work or Master of Social Work.
- Registered Social Worker with SASW preferred.
- Experience in working with children or CYP and their families.
