DESIGNATION : Senior Manager, Aging & Transition
RESPONSIBILITIES
The Senior Manager (Aging and Transition) is responsible to support the Director and Dy Director to drive and develop Aging and Transition Programme as well as other innovation pilot/trial projects that are aligned with these areas within the company.
The Senior Manager (Aging and Transition) sits within the central cluster in MINDS and is expected to work collaboratively with internal stakeholders within MINDS such as the Integrated Hubs and Community-based services. S/he is also expected to lead and manage stakeholder needs’ sensing and identify gaps and resources in service delivery in the assigned project or programmes. Upon a period of incubation and experimentation of the Aging and Transition programme in the Central Cluster, s/he is also expected to proliferate to the other clusters within MINDS and ensure the effective and efficient operations of the replication of the programmes as well.
From an applied perspective, s/he is expected to mentor staff who are operationalising the Aging and Transition programme so that there is a culture of evidence-based practice and data-driven approach to running of the specific programmes.
S/He is also expected to lead and work collaboratively with partners in the disability, eldercare, health and education sectors to build capabilities to enhance the development and operations of the Aging and Transition programmes.
S/He should possess knowledge and skills in areas of eldercare, social services and/or health. S/He should also have good project management, communication and problem-solving skills while also being able to work collaboratively with different stakeholders.
**Responsibilities**:
- Support the Director and Dy Director to drive the Aging and Transition programmes within MINDS by planning, delivering and evaluating the programmes.
- Work directly with Allied Health practitioners, educators, operational service delivery staff and caregivers to build necessary capabilities for effective operationalisation of the identified programmes within MINDS.
- Manage and lead Aging programme and Transition teams consisting of AHPs, case managers, training officers and transition support officers, where appropriate.
- To collaborate with the relevant departmental managers, principals or MINDS directors to drive evidence based practice in the identified services and programmes
- To manage service/programme outcomes evaluation projects as well as evidence-based and contemporary clinical practice and service delivery at the local level, and as directed by the Director and Dy Director (Central Cluster).
- Plan and drive pilot/trial assistive technology projects to improve outcomes of clients in the identified programmes.
- Lead the selection and implementation of appropriate solutions using analytics and visualisation for the reporting for programme outcomes.
- Oversee the identification and implementation of best-practice tools and strategies to translate raw programme data into valuable operational insights to identify current caps and future trends in the areas of aging and transition as well as propose solutions e.g. new service delivery models.
- Manage the timely and effective delivery of KPIs and milestones of grants received
- Lead the development and management of programme collaterals, resources and materials.
- Oversee inventory management of equipment and materials relevant to the identified programmes.
- Plan and manage the short-term development targets and competency building for staff in the Aging and Transition programmes.
- Lead the planning and delivery of induction and orientation for team members.
- Plan and manage professional development courses of team members.
QUALIFICATIONS
- At least a Bachelor’s degree in social services, gerontology or relevant allied health discipline (e.g. Psychology, Social Work, Occupational Therapy, Physiotherapy, Speech-Language Therapy, Nursing or other relevant social services professions) from a recognised university.
OTHER INFORMATION
- A minimum of 6 years of working experience in the field of disability, eldercare and/or other relevant areas in the social services sector.
