- OtherAdvance Care Planning (ACP), a MOH-funded programme, aims to build a culture of person-centred care (pcc) and dignity of care in Singapore through early ACP conversations. AIC, the appointed ACP National Office, is the programme manager and fund administrator to support MOH in driving strategies and initiatives to encourage take up of ACP nationally so that people can have their care preferences honoured.
You will be responsible in leading the team in:
- Drive cross ministries and AIC led publicity, outreach and citizen activation programmes to achieve uptake of ACP in the community.
- Develop strategies, infrastructure and systems to operationalise ACP processes.
- Responsible for programme and fund administration to ensure good governance and tracking of key performance indicators
- Manage multiple numbers of Community Care Organisations (CCOs)/Service providers (SPs) providing ACP services.
- Lead Secretariat for ACP Committees and/or Workgroups.
**Job Requirements**:
- Degree with at least 10 years with 3 years in a managerial capacity, preferably in the healthcare industry
- Possess strong organization skills and effective project management skills
- Possess strong analytical, critical thinking and problem solving skills and is meticulous
