- Coordinate and support ACP training programmes, ACP awareness talks and roadshows in Yishun Health.
- Monitor key performance indicators and ensure optimum utilization of resources.
- Work closely with ACP Facilitators on issues relating to ACP workflows and services.
- Work with partners in the intermediate and long term care sectors as well as other service providers.
- Work with ACP National Office on the quarterly budget, KPI reports and audits.
- Plan and execute events and projects for continued outreach efforts.
- Participate in ACP-related research and quality improvement projects in the institution.
- Provide secretariat support to the ACP Committee in the institution.
- Supervise and work with the ACP Administrative Assistant (AA) on:

Preparation and maintenance of ACP documents in the institution, and ensure information and documentation are updated for facilitators in the inpatient and outpatient settings
- Maintenance and updates of the ACP patients’ database
- Maintenance and up-to-date database of trained ACP facilitators and ACP trainers
- Coordination with patients for ACP facilitation within the institution
- **GENERAL (20%)**
- Align the department's goals with the business goals of Yishun Health.
- Plan and execute assigned projects
- Work in collaboration with key stakeholders in the evaluation and quality improvement processes on the ACP programme and projects.
- Conduct ACP discussions and documentation
- Other assigned administrative duties
