**Overall Job Responsibility**:
- SG Enable seeks to enable Persons with Disabilities (PwDs) and their families to be self reliant, living in the community with access to support services. PwDs have opportunities to participate actively in society. Caregivers are empowered with access to relevant resources and are connected with the community.**Overall Job Purpose**
This role seeks to do the following:
- Develop, pilot and review the post-18 services so that PwDs can access appropriate and relevant care services to meet their varied needs. Post-18 services can range from community and home-based care to residential living options.
- Review and develop person-centric planning so as to empower PwDs and caregivers and smoothen access to relevant support.
- Strengthen practice guidelines and standards of service delivery for post-18 services.
**Duties and Responsibilities**:
- Support SG Enable to develop and execute its mission to develop post-18 services to ensure appropriate and quality care as well as residential options for PwDs.
- Develop Enabling Services Hubs and/or relevant post-18 services which can consist of the following:
- Review policy objectives and data analysis (e.g. demand and projections), and work with MSF and relevant SGE BUs to plan the development of post-18 services
- Develop the service model(s) for relevant post-18 services;
- Develop programme evaluation framework, key performance indicators, and monitor the performance of the programme
- Work with the Service Management team to implement the pilot projects
- Review pilots to better inform longer term plans for post-18 services
- Engage stakeholders to coordinate person-centric planning such as to develop the planning goals and practices, or streamline protocols.
- Identify and develop practice guidelines and standards of care in post-18 services.
- Execute any other strategies related to the strategies of Service Development as required.
**Qualifications, Knowledge and Experience**:
- Good degree in relevant disciplines
- Relevant experience in planning, service or programme development.
- At least 6-8 years’ experience in relevant roles
- Strong communication and presentation skills
- Strong team player
- Self-driven, resourceful and able to multi-task
- Able to analyse data and familiar with data analytics
