**Responsibilities**:

- Monitor the conditions of clients and/or caregivers of persons with dementia and provide them with relevant emotional and cognitive support
- Assess the needs of clients and/or their caregivers, customize care plans, and provide relevant service linkages within the healthcare and social service sector
- Raise awareness on mental health and CREST-PDS services by engaging clients and/or their caregivers in psychoeducation efforts (e.g. training, workshops, broadcasting)
- Together with team members, plan and execute programmes and activities for clients and/or caregivers
- Ensure proper maintenance of case documentation
- Assist Programme Executive in preparing reports

**Requirements**:

- Diploma Holder
- Passionate in mental health-related social service and caregiver support
- Strong interpersonal and communication skills
- Willing to work on a weeknight and/or Sat morning (when required)
- This position is on contract basis
