Caregiver Role in Home Care

Job Responsibilities:

  • Participate in a pilot home palliative project, providing hands-on bedside care to patients.
  • Take on respite assignments as needed and assist the home care team with other nursing cases.
  • Advise care staff on medical issues or emergencies and conduct hospital or home visits to assess new clients for home care services.
  • Collaborate with the team to manage client complaints and crises.
  • Work closely with external community partners and colleagues to ensure seamless client care.
  • Maintain accurate documentation as required by regulatory bodies for financial and quality audits.

Requirements:

  • Minimum Diploma or Bachelor's Degree in Nursing.
  • Registration with the Singapore Nursing Board and a clean disciplinary record (nurses on conditional license may not apply).
  • Proficiency in Microsoft Office and comfort with IT tools used regularly during work.
  • Occasional shift work may be required to support palliative patients.
