As a Social Care Coordinator , you will play a pivotal role in ensuring the well-being of residents by providing holistic care and support. The ideal candidate should possess a strong foundation in social work, with at least one year of experience in social service/welfare work.

Key Responsibilities

  • Develop personalized care plans that address the social and emotional needs of residents.
  • Handle non-complex cases involving manageable family dynamics or social issues.
  • Prepare supporting documents for case conferences and potential transfers to step-down care facilities.
  • Facilitate discussions and documentation of Advance Care Planning (ACP) for residents when appropriate.
  • Assist residents and families in accessing essential community services and resources.

Requirements

  • Diploma holder in Social Work.
  • Minimum 1 year of relevant experience in social service/welfare work.
  • Ability to converse in local languages.
  • Good interpersonal communication skills.
