Job Overview
Social Care Coordinator
We are seeking a dedicated individual to join our team as a Social Care Coordinator. This role involves providing support and guidance to patients in need, while also contributing to the efficient operation and development of our Medical Social Services.

About the Role
The successful candidate will be responsible for gathering information to support assessments and facilitating service referrals. They will work closely with patients to connect them with suitable community resources and contribute to a comprehensive care approach that addresses both medical and psychosocial needs.

Responsibilities
  • Support patients with identifying and accessing community resources
  • Collaborate with healthcare professionals to develop and implement patient-centered care plans
  • Gather information to support accurate assessments and facilitate service referrals
  • Contribute to the efficient operation and development of Medical Social Services

Requirements
  • 6 years of experience in the social service sectors or customer service experience
  • Diploma in Social Sciences (Social Work)/ Psychology Studies/ Counselling/ Community Care or Services/ Social Service/ Social Enterprise or its equivalent
  • Able to speak local languages
  • Proficient in Microsoft Office applications
