DESIGNATION : Social Worker (Goodlife Plus)

RESPONSIBILITIES

The role is primarily in implementing Community End of Life initiatives and development plans that are congruent with the organisation's goals and the needs of the community. He /she will also part_i_cipate in designing the above-mentioned initiatives and development plans.

**Service Implementation**
- Work with the assigned community to develop community End of Life Service Workflow
- Work with ACP Coordinators to provide support in ACP referrals and enrolment and data input as needed
- Conduct talks and workshops internally and externally to raise awareness about and advocate for Advance Care Planning
- Participate in public awareness events to raise awareness about Advance Care Planning
- Promote the practice by undertaking educational, research and outreach activities as assigned from time to time
- Render professional psycho-emotional, social and spiritual care and support to clients and their significant others and, enabling them to have more meaningful and satisfying lives in the context of the situation where end of life matter is involved in the family

**Service Planning**
- Assist in the development of policies and protocol related to the delivery of community services to support clients and families facing end of life situations
- Identify upcoming trends affecting programme's success
- Identify best practices and new methodologies aimed at enhancing programme impact
- Identify service framework for programme work plans, resource requirement and budgeting
- Plan and collaborate with other organisations to achieve programme's goals

**Volunteer Management**
- Communicate appropriate programme objectives to volunteers as required
- Provide guidance to volunteers on executing effective programmes
- Develop growing team of volunteers to support various programmes
- Network and build relationships with potential partners that have many volunteers
- Establish and match links for volunteer sources within your programme, e.g. organisations with CSR project
- Identify and retain committed volunteers

**Compliance**
- Consolidate status of KPIs to Director periodically
- Prepare monthly reports to Funders and Corporate Services
- Prepare budget and ensure expenditure is within the budget
- Ensure KPIs will be met within the timeline as per funding agencies' requirement
- Support Manager, Programme in preparing annual service reports by preparing the relevant and required information

**Supervision of Team**
- Lead and support the team to meet the KPIs and smooth running of the programme
- Approve leaves and claims of the team
- Work with the Director for staff training & development and staff welfare

**Ad-hoc Duties**
- To provide any other support as assigned/required

QUALIFICATIONS
- Bachelor Degree in Social Work
- At least 3 years of experience as an Allied Health Professional
- Certification in Advance Care Planning Facilitation (General and Preferred Plan of care) and experience in conducting ACP discussions would be an advantage
