Senior Care Programme Developer

We are seeking a dedicated professional to join our team in developing innovative programmes to support the needs of our community. The role involves coordinating social health services, connecting seniors to relevant service providers, and facilitating care for frail and vulnerable seniors.
Key Responsibilities:
- Develop and pilot workflows, protocols, and processes to facilitate social-health integration
- Establish linkages and protocols between different community providers to manage clients across multiple social-health services
- Review and track the effectiveness of inter-agency workflow in facilitating care transition and provision
- Design and implement new initiatives to meet the social and health needs of the community.
Requirements:
Applicants should possess a Bachelor/Graduate Diploma in Social Work or relevant fields (Psychology, Community Work) with at least 2-3 years of relevant working experience in eldercare or healthcare settings.
Strong interpersonal and communication skills, as well as high proficiency in written and spoken English, are essential for this role.
