Description Overview Programme Lead @ Ren Ci Woodlands role at Ren Ci HospitalLocation: WoodlandsJob Category: AdministrationPosting Date: 9 Oct 2025Responsibilities Support Ren Ci Hospital and collaborate with its working partners to organise and operationalise the Integrated Community Care Provider (ICCP) programme in the sub-region.Monitor progress of ICCP programme and ensure timely adherence to targets and submissions of reports to Agency of Integrated Care (AIC).Support the sub-region ICCP in the day-to-day coordination, administrative and operational duties.Perform daily work duties according to Ren Ci Hospital’s standard operating procedures (SOP) and/or according to the requirements set forth by ICCP Committee(s).Work with leadership team of the sub-regional ICCP organisations to align goals, establish roles, and develop strategies and workplans for the sub-region to achieve ICCP deliverables.Collaborate with centre managers and ground teams to implement the ICCP workplan, monitor and report on progress, and address challenges.Develop and trial joint referral workflows, care coordination deliverables and other outcomes arising from the programme with ICCP organisations.Engage and build networks and partnerships with community care providers, regional health systems, grassroot organisations and other stakeholders.Plan and coordinate ICCP meetings, set agendas, and prepare meeting minutes.Collect and consolidate information required for ICCP quarterly progress reports and other required reports.Ensure that endorsed reports and claim forms are submitted to AIC in a timely manner.Represent the ICCP in meetings convened by AIC as required, and report back to the leadership teams of partner organisations.Participate in relevant training programmes to acquire skills and knowledge that enhance work performance.Perform other duties as required or instructed by the reporting officer or Head of Department.Qualifications Diploma or degree in healthcare-related field or other relevant field of study.At least 2 years of working experience in the community care sector.Basic proficiency in Microsoft Word, Excel and PowerPoint, and other computer skills.Strong planning, coordination and secretariat skills, including planning meeting agendas, minutes-writing and report-writing.Able to work well independently and as a team player.Good interpersonal, communication and networking skills to forge partnerships with community partners.Good time-management, well-organised, able to multi-task and adapt to working demands.Monday to Friday (Office hours) with ability to work on weekends when needed.Seniority level Entry levelEmployment type Full-timeJob function Consulting, Information Technology, and SalesIndustries Hospitals and Health Care #J-18808-Ljbffr
