Job Title: Mental Health Support Specialist
We are seeking a highly skilled Mental Health Support Specialist to join our team. As a key member of our healthcare team, you will play a vital role in providing mental health support services to clients and their caregivers.
Job Summary
The ideal candidate will have a strong background in social work and experience working with clients with mental health issues and/or dementia. You will be responsible for conducting mental health assessments, providing home-based direct interventions, and supporting clients and caregivers with mental health issues and/or dementia.
You will also be responsible for developing and implementing service planning documents and manuals, including set up requirements and processes for outreach, screening, mental health information, service linkage, and follow-up.
Key Responsibilities
- Conduct mental health assessments and provide psychoeducation, case management, and counselling services to clients and caregivers.
- Develop and implement service planning documents and manuals, including set up requirements and processes for outreach, screening, mental health information, service linkage, and follow-up.
- Maintain accurate and timely client records for submission to the Community Intervention Team (COMIT) programme.
- Support in the development of COMIT service planning documents and manuals.
- Keep and maintain client and programme data and documents in accordance with the COMIT programme requirement and personal data protection guidelines.
Requirements
- Degree or postgraduate diploma in Social Work.
- At least 2-3 years of experience in community or hospital settings.
- Good team player with excellent interpersonal, communication, writing, and IT skills.
- Familiarity with the healthcare environment is an added advantage.
- Ability to communicate with vernacular languages-speaking clients.
Preferred Qualifications
- Dementia or mental health training or working experience.
- Experience using psychotherapy with mental health patients.
