Medical Social Worker Position
We are seeking a dedicated and compassionate social worker to join our team. In this role, you will be responsible for conducting thorough assessments and developing effective plans to support the well-being of residents and clients.
- Conduct in-depth psycho-social and financial assessments, identifying areas for improvement and providing recommendations for clinical interventions and case management.
- Develop and implement individualized care plans that address the unique needs of each resident or client, working closely with families and multidisciplinary teams to ensure seamless coordination of care.
- Collaborate with healthcare professionals to facilitate family conferences and attend regular multi-disciplinary meetings to discuss progress and make informed decisions.
- Manage complex case files and maintain accurate documentation using integrated systems, ensuring timely submission of reports and adherence to regulatory requirements.
- Utilize exceptional interpersonal, organizational, and communication skills to build strong relationships with colleagues, residents, and clients, effectively communicating care plans and facilitating open dialogue.
- Maintain up-to-date knowledge of relevant laws, policies, and procedures, applying this expertise to inform practice and ensure compliance.
Key Requirements:
- Minimum degree in Social Work from an accredited institution.
- Registration with The Social Work Accreditation and Advisory Board (SWAAB) is mandatory.
