Job Description:
This role is a crucial part of the United Primary Care Network, focusing on patient engagement to achieve key objectives. As a Patient Engagement Executive, you will work independently and collaboratively with a team of like-minded staff to deliver high-quality care to patients.
You will be responsible for engaging patients in various programs, including Healthier SG (HSG), Chronic Disease Management Programmes (CDMP), diabetic screenings, nurse counselling, and community care. Additionally, you will coordinate appointments, provide reminders, gather feedback, and offer program-related information through outreach calls and follow-ups.
- Engage patients in PCN programmes to encourage participation.
- Refer eligible patients to ancillary services such as Diabetic Foot Screening (DFS), Diabetic Retinal Photography (DRP), Nurse Counselling (NC), and Spirometry (Spiro).
- Conduct outreach calls and follow-ups to coordinate appointments and provide programme-related information.
Key Responsibilities:
The successful candidate will have the following responsibilities:
- Maintain accurate patient records and update data in internal tracking systems.
- Address patient queries and concerns with professionalism, empathy, and confidentiality.
- Assist in preparing reports, tracking key performance indicators (KPIs), and ensuring compliance with MOH documentation requirements.
Requirements:
To be considered for this role, you should have:
- Diploma in a relevant field with 2 years of working experience.
- Familiarity with the way of working of a clinic.
- Relevant experience in call center, customer service/support hospitality or healthcare industry is advantageous.
- Team player, good interpersonal and communication skills.
