Patient Care Coordinator

We are seeking a skilled and compassionate Patient Care Coordinator to join our nursing team. As a key member of the team, you will be responsible for delivering high-quality patient care that meets the highest standards of professionalism and compassion.
Key Responsibilities:- Patient Care Delivery: Collaborate with Registered Nurses to deliver exceptional patient care, including monitoring, documenting, and reporting on patient conditions and progress.
- Liaison Services: Act as a liaison between patients and healthcare professionals, escalating any unusual observations or concerns.
- Equipment Preparation: Prepare equipment and trolleys for ward-based procedures, such as blood cultures and lumbar punctures.
- Perioperative Care: Provide perioperative care, including preoperative preparation and postoperative recovery support.
- Life Support: Perform basic life support and cardiopulmonary resuscitation (CPR) as required.
- Daily Living Assistance: Assist patients with daily living activities, such as bathing, dressing, and toileting, while promoting skin integrity and mobility.
- Critical Thinking: Exercise critical thinking skills to resolve complex problems and make sound judgments.
- Safe Care Practices: Practice safe care within your scope of competence and adhere to proper equipment use and maintenance.
- Empathy and Communication: Display empathy, a positive attitude, and a caring demeanor towards patients and colleagues.
- Education: Completion of a recognized healthcare program or equivalent experience.
- Experience: Previous experience in a healthcare setting, preferably in a patient care role.
- Skills: Strong communication and interpersonal skills, ability to work effectively in a team environment.
