Senior Patient Care Coordinator

The Senior Patient Care Coordinator plays a pivotal role in supporting the delivery and operations of patient care services. Working closely with the Clinical Nurse and other team members, the Coordinator ensures the seamless functioning of administrative processes, client care coordination, and service quality.
Key Responsibilities:- Client Admission & Discharge Coordination: Conduct thorough intake assessments, including comprehensive financial counselling and service agreement management. Ensure timely and accurate documentation throughout the admission and discharge process for patient referrals.
- Service Monitoring & Staff Rostering: Oversee the daily provision of personal care services to patients. Coordinate staff scheduling, including rostering and leave management, to ensure adequate coverage and continuity of care.
- Operations Support & Administrative Management: Provide day-to-day operational support and maintain comprehensive and up-to-date records for all patients, including filing, tracking, and updating documentation as needed.
- Staff Development Support: Assist in the implementation of training programs and competency frameworks for care staff to enhance service quality and staff performance.
- Client & Caregiver Engagement: Obtain regular feedback from patients and caregivers to assess satisfaction and identify areas for improvement. Address concerns and escalate issues appropriately to ensure patient well-being.
- Custodial Care Support: Provide hands-on support in custodial care when required, ensuring patients' comfort and safety.
- Degree in Nursing or relevant field.
- Excellent communication and interpersonal skills.
- Organized and meticulous individual who is a team player.
- Proficient in Microsoft Office.
- Strong understanding of working with elderly patients.
