  • The successful candidate will be responsible for conducting thorough assessments of clients through home visits or social visits, evaluating their social and emotional needs.
  • A strong focus on coordination and advocacy, this role requires the ability to navigate various parties, including community organizations, government agencies, and individuals, to advocate for clients' benefits and welfare.
  • Providing education and support to patients and families about illnesses, treatment plans, and available resources, this specialist will empower them in managing their health effectively.
  • Developing comprehensive care plans that assess each patient's unique condition, combining medical diagnoses and personal interviews, this role ensures that necessary support is tailored to each patient's health status, personal strengths and weaknesses, and emotional state.
  • Evaluating the effectiveness of services and resources provided to patients and adjusting plans accordingly, this specialist will follow up with patients throughout their diagnosis, treatment, and recovery process to assess their mental and emotional well-being and provide ongoing support.
Required Skills and Qualifications:

Recognized Degree in Social Work
