Patient Care Coordinator

As a dedicated Care Specialist, you will play a pivotal role in delivering exceptional patient care. This role is centered around providing high-quality geriatric and neurological physiotherapy services.
The successful candidate will work collaboratively with a multi-disciplinary team to provide patient-centered care, participating in patient consultations and supervising Therapy Assistants.
A key aspect of this job is accurately documenting patient physiotherapy records and developing treatment plans for patients with a range of injuries and conditions.

To excel in this position, you will require:
