Job Overview:

Main Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify patients' needs, strengths, and challenges.
  • Collaborate with patients, families, and relevant agencies to create and implement tailored care plans that ensure seamless transition from healthcare facility to home.
  • Participate actively in multidisciplinary meetings to discuss and coordinate patient care.
  • Organize and participate in case conferences with relevant organizations and community agencies to enhance patient care and service coordination.
  • Provide emotional support and counseling services to patients and their families, addressing concerns and promoting resilience.
  • Lead group sessions for patients or caregivers to offer shared learning and support.
  • Assist patients and their families in accessing available community services and support to meet their needs.
  • Conduct home visits and follow-up sessions to address ongoing concerns and monitor progress.
  • Maintain accurate and timely documentation of patients' care plans, interventions, and coordination efforts within medical systems, ensuring compliance with healthcare regulations.

Required Skills and Qualifications:
  • Degree in Social Work, preferably with 1-2 years' experience in the field.
  • Experience in a healthcare or social services agency is advantageous.
  • Registration with the Social Work Accreditation Board is preferred.
  • Prioritizes an interest in working with seniors and their caregivers.
  • Able to work independently.
