Job Role: Elderly Care Specialist

\

The Elderly Care Specialist provides casework, interventions, and care coordination services to older persons and their caregivers within the home and community settings. This role supports ageing-in-place by addressing the psychosocial, emotional, and environmental needs of clients, facilitating access to care, and working in collaboration with our multidisciplinary team and external partners.

\Key Responsibilities\
  • Casework and Interventions: Conduct intake and comprehensive biopsychosocial assessments to evaluate clients' social, emotional, and financial needs. Develop, implement, and monitor individualised case and care plans to address psychosocial risks, caregiver stress, and safety concerns.
  • Care Coordination and Service Integration: Collaborate with allied health professionals, nurses, and care aides to ensure holistic care planning. Liaise with external stakeholders to mobilise social and financial support.
  • Programme Development & Group Work: Design and implement group work intervention plans to meet client needs. Supervise facilitation of group sessions and evaluate effectiveness.
  • Community Development & Outreach: Conduct environmental scans and community needs assessments. Mobilise community resources and initiate development projects.

This role also involves participating in research and service improvement initiatives, building and maintaining strong relationships with community partners and stakeholders, and supporting our community outreach efforts to identify at-risk elderly in the community.
