Nephrology Care Coordinator Job Opportunity

This role involves providing exceptional customer service, support and coordinating clinical care for renal patients in both inpatient and outpatient settings.

  • Early Intervention: Assist patients with chronic kidney disease (CKD) to prevent disease progression through education, follow-up appointments, evaluation of test results, and discussions with primary physicians regarding appropriate interventions.
  • Advanced CKD Management: Coordinate the transition of patients from non-dialysis dependent to dialysis dependent by providing pre-dialysis counseling, access creation coordination, follow-up on patient decisions regarding modality, Voluntary Welfare Organisation (VWO) applications and approvals, and interim haemodialysis centre arrangements.
  • Liaison Services: Collaborate with vendors, dialysis centres, patients, and caregivers related to end-stage kidney disease.
  • Peritoneal Dialysis Competency: Conduct training sessions for patients opting for peritoneal dialysis.
  • Advance Care Planning: Facilitate advance care planning discussions for pre- and post-initiation dialysis patients in both inpatient and outpatient environments.
  • Patient Follow-Up: Monitor and track patients' progress, including hepatitis B vaccination, anaemia management, calcium-phosphate control, and nutrition status.
  • Workload and Outcome Monitoring: Track statistics on workload and quality outcomes.
  • Dialysis-related Issues: Identify and address issues related to treatment adherence, cardiac complications, anaemia, and nutrition, particularly among patients on the renal transplant waitlist.
  • Care Coordination: Coordinate the care of patients awaiting renal transplants, requiring yearly reviews at transplant centres in tertiary hospitals.
  • Inpatient Dialysis Complications: Monitor the outcomes of inpatient dialysis-related complications and hospitalisations.
  • Interdisciplinary Collaboration: Work closely with nephrologists, surgeons, dialysis nurses, medical social workers, dietitians, and pharmacists to ensure high-quality patient care.
  • Professional Development: Participate in seminars and workshops on kidney disease.
  • Reporting and Documentation: Prepare monthly reports, track workload, key performance indicators, and balance scorecards; maintain systematic, orderly, and confidential patient documents for audit purposes.
  • Statistics Submission: Submit HD and PD patient statistics to the National Registry Disease Office (NRDO).
Requirements and Qualifications
  • Bachelor's Degree in Nursing.
  • Minimum 2 years working experience, preferably with renal-related expertise.
  • Basic IT knowledge required; proficient PC skills an advantage.
  • Possess a positive attitude and consistently deliver high-quality performance.
  • Strong interpersonal and communication skills.
  • Able to work independently and proactively implement change.
