Job Title: Home Care Specialist

About the Role:

  • You will work closely with patients and their families or caregivers to develop and implement comprehensive care plans that meet their individual needs.
  • Responsibilities include providing safe, competent, and compassionate nursing services, upholding industry standards in documentation, treatment, patient monitoring, and infection control.
  • You will communicate effectively with healthcare teams, physicians, and other stakeholders to ensure seamless care coordination and continuity.
  • Performing essential nursing tasks such as medication administration, wound dressing, and vital sign monitoring as required.

Key Responsibilities:

  • Provide personalized care to patients in their homes.

  • Uphold industry standards in documentation, treatment, patient monitoring, and infection control.

Requirements:

  • Registered with Singapore Nursing Board.
  • Diploma in Nursing or equivalent qualification.
  • Experience in bedside nursing with strong interpersonal skills.
