Home-Based Elderly Caregiver

This is a unique opportunity for a qualified and experienced staff nurse to join our mobile clinic team. As a staff nurse, you will be working closely with doctors and social workers to provide home-based nursing services to frail and elderly individuals in the community.
Key Responsibilities:- Conduct regular home visits and perform comprehensive nursing assessments of clients
- Triaged client conditions and respond promptly to address any issues that may arise
- Collaborate with the interdisciplinary team to develop and implement client care plans
- Explain care goals and plans to clients and caregivers to ensure active participation
- Evaluate outcomes of interventions and revise care plans as needed
- Manage and monitor clients' medical and nursing needs as required
- Maintain accurate documentation of clients' progress and outcomes of care provision
- Assist doctors in charge and perform nursing procedures as needed
- Provide education and training to clients, caregivers, and team members as necessary
- Professional certificate or diploma in nursing, preferably with training in gerontological nursing
- State registration with the Singapore Nursing Board and possession of a valid practicing certificate in Singapore
- At least 2-3 years of experience in the social services or eldercare sector
- Extensive clinical knowledge and skills
- Ability to work with the elderly and their caregivers
- Flexibility, energy, and multitasking skills
- Excellent communication and interpersonal skills
- Bilingual in at least two local dialects
- Indicative salary commensurate with qualifications and experience
- Working location in the West Region
