As a Dedicated Care Provider, you will play a vital role in delivering high-quality home-based nursing services to frail and home-bound elders, ensuring that our clients receive the best possible care. Your primary responsibility will be to conduct regular home visits and comprehensive nursing assessments for elderly clients, identifying their medical and nursing needs and developing personalized care plans to meet those needs.

Your expertise will be essential in collaborating with healthcare professionals to develop and implement these care plans, communicating effectively with clients and caregivers to ensure active participation and evaluation of intervention outcomes. You will also be responsible for maintaining accurate documentation of client progress and care outcomes, as well as assisting doctors and performing nursing procedures as required.

Key Responsibilities:

  • Conduct regular home visits and comprehensive nursing assessments for elderly clients
  • Triage client conditions and respond promptly to address any issues
  • Collaborate with healthcare professionals to develop and implement personalized care plans
  • Communicate care goals and plans to clients and caregivers to ensure active participation
  • Evaluate intervention outcomes and revise care plans as necessary
  • Manage and monitor clients' medical and nursing needs
  • Maintain accurate documentation of client progress and care outcomes
  • Assist doctors and perform nursing procedures as required

Requirements

  1. Professional Certificate or Diploma in Nursing, preferably with gerontological training
  2. State registration with the Singapore Nursing Board and valid Practising Certificate
  3. 2-3 years of experience in social services or eldercare
  4. Extensive clinical knowledge and skills
  5. Ability to work with the elderly and caregivers
  6. Flexible, energetic, and able to multitask
  7. Excellent communication and interpersonal skills
  8. Bilingual in 2 or more local dialects
