Key Responsibilities
- Conduct comprehensive biopsychosocial assessments to evaluate clients' social, emotional, and financial needs, ensuring a deep understanding of each individual's situation.
- Develop, implement, and monitor individualised case and care plans to address psychosocial risks, caregiver stress, and safety concerns, promoting holistic support and outcomes.
- Provide counselling, emotional support, bereavement care, and facilitate appropriate individual and/or family interventions, fostering a sense of security and well-being among clients.
- Perform home visits, means testing, and financial counselling as required, demonstrating empathy and understanding towards clients' unique circumstances.
- Assist clients in accessing financial aid schemes and community services through timely referrals, bridging gaps in care and support.
- Maintain accurate and prompt documentation of interventions, progress, and reports, ensuring transparency and accountability in your work.
Programme Development & Group Work
- Design and implement group work intervention plans to meet client needs, leveraging the power of collective support and shared experiences.
- Supervise facilitation of group sessions and evaluate effectiveness, refining your approach to better serve clients and communities.
- Identify service gaps and lead development of new programmes, driving innovation and improvement in care and support services.
- Oversee programme implementation and outcomes evaluation, ensuring the successful delivery of high-quality services that make a meaningful difference in people's lives.
Community Development & Outreach
- Conduct environmental scans and community needs assessments, gathering insights into local contexts and priorities.
- Mobilise community resources and initiate development projects, forging partnerships and collaborations that amplify impact and enhance care services.
- Build and maintain strong relationships with community partners and stakeholders, fostering trust and cooperation that underpins effective care delivery.
- Organise and lead crisis management efforts with external agencies, responding promptly and effectively to emerging challenges and needs.
