As a skilled professional in care coordination and social work, you will play a vital role in supporting older persons and their caregivers within the home and community settings. The Care Coordinator and Social Worker will be responsible for addressing the psychosocial, emotional, and environmental needs of clients, facilitating access to care, and working in collaboration with a multidisciplinary team.
Key Responsibilities
  • Conduct comprehensive biopsychosocial assessments to evaluate clients' social, emotional, and financial needs, ensuring a deep understanding of each individual's situation.
  • Develop, implement, and monitor individualised case and care plans to address psychosocial risks, caregiver stress, and safety concerns, promoting holistic support and outcomes.
  • Provide counselling, emotional support, bereavement care, and facilitate appropriate individual and/or family interventions, fostering a sense of security and well-being among clients.
  • Perform home visits, means testing, and financial counselling as required, demonstrating empathy and understanding towards clients' unique circumstances.
  • Assist clients in accessing financial aid schemes and community services through timely referrals, bridging gaps in care and support.
  • Maintain accurate and prompt documentation of interventions, progress, and reports, ensuring transparency and accountability in your work.

Programme Development & Group Work
  • Design and implement group work intervention plans to meet client needs, leveraging the power of collective support and shared experiences.
  • Supervise facilitation of group sessions and evaluate effectiveness, refining your approach to better serve clients and communities.
  • Identify service gaps and lead development of new programmes, driving innovation and improvement in care and support services.
  • Oversee programme implementation and outcomes evaluation, ensuring the successful delivery of high-quality services that make a meaningful difference in people's lives.

Community Development & Outreach
  • Conduct environmental scans and community needs assessments, gathering insights into local contexts and priorities.
  • Mobilise community resources and initiate development projects, forging partnerships and collaborations that amplify impact and enhance care services.
  • Build and maintain strong relationships with community partners and stakeholders, fostering trust and cooperation that underpins effective care delivery.
  • Organise and lead crisis management efforts with external agencies, responding promptly and effectively to emerging challenges and needs.
