Cancer Patient Navigator

At our organization, we are seeking a dedicated and compassionate individual to fill the role of Cancer Patient Navigator. As a key member of our team, you will be responsible for providing comprehensive support to patients with cancer, ensuring they receive the highest level of care throughout their journey.

The Cancer Patient Navigator will serve as the primary point of contact for patients, facilitating communication between healthcare providers, addressing patient concerns, and promoting seamless coordination of care. This position requires a strong understanding of cancer treatment options, medical terminology, and patient-centered care principles.

Key Responsibilities:

  • Develop and maintain accurate patient records, including medical history, treatment plans, and progress notes.
  • Coordinate appointments, tests, and procedures, ensuring timely and efficient delivery of services.
  • Provide emotional support and counseling to patients and their families, addressing concerns and fears related to diagnosis, treatment, and recovery.
  • Collaborate with healthcare teams to develop personalized care plans, incorporating the patient's preferences, values, and needs.
  • Stay up-to-date on the latest cancer treatment options, guidelines, and research, applying this knowledge to inform patient care decisions.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient outcomes and experiences.

Requirements:

  • Bachelor's degree in a relevant field (e.g., nursing, social work, healthcare administration).
  • Minimum 2 years of experience in a healthcare setting, preferably in oncology or patient navigation.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong problem-solving and critical thinking skills, with the ability to navigate complex healthcare systems and resolve issues efficiently.
  • Proficiency in electronic health record systems and other software applications relevant to the role.
