Residential Case Manager Job Opportunity

Job Summary: We are seeking a highly skilled Residential Case Manager to join our team. As a key member of our case management unit, you will be responsible for conducting pre-emplacement interviews with residents, developing case plans, and implementing, reviewing, and modifying them as needed.

Key Responsibilities:

  • Conduct thorough pre-emplacement interviews to understand resident needs and develop personalized case plans.
  • Implement, review, and modify case plans to ensure effective support and progress.
  • Prepare and maintain Individualised Service Plans (ISPs), progress reports, and closure documents.
  • Provide casework and counseling services to residents and their families, ensuring they receive the necessary support and guidance.
  • Refer residents and their families to relevant agencies for further intervention and resources.
  • Ensure that our halfway house program meets its mission, objectives, and procedures, providing high-quality services to our residents.

Requirements:

  • Hold a Degree or Post Graduate Diploma in Social Work or a related field from an accredited institution.
  • Registered as an Accredited Social Worker or Social Service Practitioner is an advantage.
  • Minimum 2-3 years of relevant casework and counseling experience in the field, preferably working with offenders.
  • Excellent interpersonal and communication skills, both written and oral.
  • Ability to work independently and as part of a team.
  • Self-motivated with a drive to improve and develop processes.
  • Proficient in Microsoft Office applications, including Word and Excel, and basic statistical analysis.
