Senior Resident Case Manager

We seek an experienced and skilled Residential Case Manager to join our team.
- Conduct comprehensive needs assessments using the Child and Adolescent Needs & Strengths (CANS) and Structured Decision Making (SDM) tools to identify residents' strengths and areas for improvement.
- Develop and implement individualized care plans that address residents' unique needs and goals, incorporating strategies from evidence-based practices.
- Cultivate partnerships with various stakeholders to ensure seamless implementation of care plans and interventions, promoting a collaborative approach to service delivery.
- Foster connections between residents and community resources, facilitating their reintegration into their communities.
As a Residential Case Manager, you will lead case conferences to review interventions and make data-driven decisions, ensuring the highest quality of care. You will also maintain accurate and up-to-date records, guaranteeing compliance with regulatory requirements.
In addition to your core responsibilities, you will play a key role in planning and facilitating group work interventions, supporting the implementation of standards of care, and identifying opportunities for process improvements in our service delivery models.
