Job Title: Social Care Advocate

This role is responsible for providing professional support to patients and their families. The successful candidate will be a key member of our healthcare team, working closely with patients, family members, and other healthcare professionals to deliver exceptional care.

Key Responsibilities:

  • Develop personalized care plans for clients in collaboration with multidisciplinary teams.

  • Provide emotional support through counseling sessions, using active listening skills and empathy to build trust and rapport.

  • Collaborate with healthcare professionals and family members to create effective care strategies that meet the unique needs of each client.

  • Perform assigned projects by the supervisor, demonstrating flexibility and adaptability in a fast-paced environment.

Qualifications & Requirements:

  • Hold a Diploma/Degree in Social Work or equivalent, with a strong understanding of social care principles and practices.

  • Be registered with relevant professional bodies, such as the Singapore Association of Social Workers.

  • Have minimum 2 years working experience in healthcare/ILTC sector, preferably with experience in clinical supervision and community outreach.

  • Be familiar with healthcare systems to liaise with various government agencies and stakeholders.
