Job Title: Senior Social Worker

The primary function of this position is to conduct comprehensive case assessments and develop individualized care plans for individuals with dementia and their caregivers.

Key Responsibilities:
  • Conduct thorough assessments to determine the needs of individuals with dementia and their caregivers
  • Develop and implement holistic care management plans to ensure optimal support
  • Provide interventions such as psychoeducation, supportive counseling, and service referrals
  • Liaise and collaborate with multidisciplinary professionals and external service providers
  • Document all case recordings and relevant documentation
  • Participate in team meetings and case conferences as required
  • Assist in data collection and statistics for program evaluation and reporting purposes
  • Plan and facilitate caregiver support group sessions
  • Support outreach events and talks
Requirements:
  • Degree in an allied health profession, social work, or psychology (senior position may be considered)
  • Excellent oral, written, and interpersonal communication skills
  • Ability to facilitate group sessions (added advantage)
  • Able to work on occasional Saturdays and weekday evenings
  • Possesses a passion for dementia care and working with people
  • Prior experience in dementia care (added advantage)
