  • Job Responsibilities:">
    • Review eligibility of persons referred into the service to ensure they meet the necessary criteria.">
    • Onboard eligible persons into the digital platform, ensuring a smooth and seamless experience.">
    • Monitor the health status of persons in the service remotely, using your analytical skills to identify areas of concern.">
    • Engage with persons enrolled in the service, building strong relationships and trust.">
    • Follow up with patients and escalate to the assigned healthcare worker if needed, as per stipulated care protocol.">
    • Ensure critical information is communicated within the team, promoting effective collaboration and teamwork.">
    • Provide administrative support, handling tasks efficiently and effectively.">
  • Requirements:">
    • Prior experience in mental healthcare and/or digital technology is advantageous, but not essential.">
    • A passion for mental health and a commitment to making a positive impact are essential.">
    • Diploma in any discipline or equivalent qualification.">
    • Proficient in Microsoft Office applications, with excellent organizational and communication skills.">
