Job Description

We are seeking a skilled and compassionate healthcare professional to join our team as a Rehabilitation Therapist. In this role, you will be responsible for conducting thorough assessments and implementing tailored therapy interventions that prioritize individual client needs.

Your key responsibilities will include:

  • Conducting occupational/physiotherapy assessments during client admissions, periodic care reviews, and at discharge.
  • Selecting and implementing outcome measures to gauge therapy progress and effectiveness.
  • Collaborating with clients and stakeholders to develop personalized goals.
  • Designing evidence-based therapy interventions that prioritize client needs and preferences.
  • Maintaining accurate documentation of assessment results, therapy plans, interventions, and outcomes.
  • Effectively communicating assessment findings, intervention plans, and discharge strategies to clients and relevant parties.
  • Ensuring proper utilization and client training on rehabilitation equipment.
  • Providing recommendations for assistive devices and equipment as needed.
  • Developing comprehensive discharge and continuity of care plans.
  • Fostering community integration through strategic partnerships.
  • Contributing actively to case discussions within the care team.
  • Offering guidance and support in training Therapy Assistants.
Requirements
  • Degree in Physiotherapy or Occupational Therapy
  • Registered with Singapore Allied Health Professional Council (AHPC)
